• Health Form
• Co-Curricular Selection Form
• Other required documents

Documents received by school & checked for completeness
Student may start school

Note: If academic records are not in English, they have to be translated to English and certified

A photocopy of the applicant’s Birth Certificate
A photocopy of the applicant’s I.C. (if applicable)
For Foreign Students
A photocopy of the applicant’s Birth Certificate, Household Register or Family Certificate
A photocopy of the applicant’s Passport showing the name, passport number & date of birth
A photocopy of the father’s Passport showing the name, passport number & date of birth
A photocopy of the mother’s Passport showing the name, passport number & date of birth
Two Recent Passport-Size Photographs
Please paste one of the photographs on the Student Application Form
Please enclose the other photograph with the applicant’s name written on the back
